STARTING POINT - THE CLUB SECRETARY

The world requires some form filling in and Slimbridge YFC is regrettably no different. We require all new members and their parents/guardians to complete the following registration details:

• SYFC Registration 2007 - 2008
• Medical Form 2007 - 2008
• SYFC Code of Conduct (Parents)
• SYFC Code of Conduct (Players)

Sign On fees are £25 and £15 (post XMAS). Match Fees are £1.50. Kit (excluding shin pads and football boots) are provided by the club for return when you leave in reasonable condition. Please note that Sign On fees are non refundable.

And just when you thought you had finished, the Leagues need a bit too! With the exception of the U8s everybody else needs to fill in these extra bits.

STROUD & DISTRICT (U13 and U16 w.e.f 31st Aug 07)

In order for an individual to play in either the Stroud & District Youth League (currently the U13s and U16s) you must complete the leagues registration form and provide a copy of your passport or NHS card or Birth Certificate, together with two passport size photographs to qualify for a Player's I.D Card

If you already have an ID card it must be returned to your manager with the league registration documents such that it cna be revalidated by S&DYL. 11 players must be re-registered by no later than 1st Sep 2007.

GLOUCESTER YFL (U14s w.e.f 31 Aug 07)

Just to complicate it further - Our Under 14s play in the Gloucester Youth Association Football League and they require [tbc identity documents] and:

MID GLOUCESTER MINI SOCCER (U9s, U10s & U11s)

Similarly if you are in the U9's or Under 10's age group (Sep 2006) then you will need to complete the Mid Gloucestershire Mini Soccer League registration form and also provide a photocopy of your birth certificate or NHS Card or Passport.
